It is the patient's responsibility to check their insurance plan coverage. To avoid unpleasant and expensive surprises, we strongly encourage our patients to carefully research the specific provisions of their health insurance coverage. It is the patient's responsibility to know the specific requirements of their coverage, and to inform our office of all special requirements. It is important that you bring your insurance card to every appointment. If you change insurance companies or change your address, phone number, etc., please contact our office as soon as possible so we can make the necessary changes to your file. Furry friends become more than pets when a significant bond with their “person” is established. A specific bread is something to consider when choosing a furry support friend. The distinction of Emotional Support Animal versus a Service Dog is important to understand. Both designations require ADA verification. This means that a disability acknowledged by Social Security, e.g. a statement clarifying disability or a medical doctor statement clarifying you do have a disability. Training for a support animal can be provided by the owner. This type of training versus a certified service animal trainer can be a better economic fit. However, this does not necessarily certify that your Furry Friend is a service animal that will exclude you from the guidelines set by establishments. While questions such as why you require your Furry Friend to accompany you on medical appointments or in the community. These questions are limited to what service the animal provides. A specific disability question in some instances is not appropriate. Many organizations have begun to enforce stricter polices regarding the accompaniment of a Furry Friend regardless of presenting certification of one's service animal. I do have certified Service/Emotional support dog, her name is Majeek and she is a pure bread Blue Pomeranian. This breed ad is an excellent choice for either type of support animal as they are very empathic bonding. The following are establishments that will not permit a Service Animal or Emotional Support animal and do not accept certification provided by the owner that supports the ADA guidelines and our Therapist/Counselor letter and or/ medical provider. Providence Medical Group-Neurology: The office manager will instruct the Medical Assistant to inform you future appointments attended in person with your Furry Friend the appointment will be canceled and you will be charged a late cancel fee. Vancouver Radiology: This organization will advise upon check in that your scheduled service will require rescheduling to a time and day that you will not have your Furry Friend in attendance. Peace Health Medical Group Surgery: The office manager will advise you that he will call security to have you physically removed as they do not acknowledge, nor will they consider reading any documentation to verify you meet ADA guidelines.
Emotional Support & Service Animals:
